There are many different ways that the liposuction procedure is performed today are discussed below. The main differences between these techniques are as follows:

  • Solution has to be injected under the skin to perform a liposuction. These procedures will vary in the amount of the solution that is used for this.
  • Some additional devices may be needed to assist in performing the liposuction and this varies from technique to technique.

Due to their poor safety records, some of the older techniques are no longer used.
   Technique 1 – Dry:
This technique is no longer used. It would have required general anesthesia. It was called the dry technique because of the fact that injection of a local anesthetic into the fat cells was not performed before the liposuction was performed. This often caused excessive blood loss and therefore is no longer performed. Approximately 30% of the tissue removed during this type of procedure was blood.

   Technique 2 – Wet:
This technique also required a general anesthetic to be administered. This technique has also been discarded. For this type of procedure, around 100 ml of local anesthetic with epinephrine was injected into the fat cells. This technique caused far less blood less than the dry technique but was still considered unsafe. With this technique, about 15-20% of the tissue being removed consisted of blood.

   Technique 3 – Super Wet:
This technique also requires a general anesthetic. For this technique, a volume of dilute local anesthetic is injected, but this is less than half the volume required for the tumescent technique. Surgical blood loss is about 8% of the fluid removed, which is significantly less than the wet technique, but greater than the blood loss in the tumescent technique.

   Technique 4 – Tumescent :
Tumescent Liposuction requires the injection of a large volume of very dilute local anesthetic that contains both lidocaine and epinephrine. This amount of anesthetic causes the fat cells to become “swollen and firm” or “tumescent”, hence the name. This is the only procedure that permits liposuction with only local anesthesia. With this technique, only about 1% of the fluid removed during the procedure is blood.

Technique 5 –Power Assisted Liposuction (PAL):
PAL devices are also now available. They use power supplied by an electric motor or compressed air. They either produce a rapid in and out movement or a spinning movement of the attached liposuction cannula. These are purported to make the procedure easier for the surgeon. Though some surgeons advocate the use of Power Assisted Liposuction, many remain skeptical about its advantages. It should be noted that there are still no objective scientific publications that support the claims made by the manufacturers of these devices.

Tumescent Liposuction – The procedure itself:
Large volumes of a solution are injected into the targeted fat areas. This solution is usually a local anesthetic with epinephrine. Epinephrine causes the shrinkage of blood capillaries and helps to minimize any blood loss. A small incision is made in the skin, and a small tube called a “cannula” attached to a vacuuming device is then inserted into the fatty deposits. Back and forth movements are used and the fat is sucked through the tube into a sterile collection unit. An elastic compression garment placed on the treated area to assist the skin in contracting and healing, resulting in a more sculpted look and more attractive contour

   Benefits of the Tumescent Technique:
The several benefits of this technique include less bleeding, fewer skin irregularities, less bruising, and a speedier recovery. It is also performed as an outpatient procedure, thus avoiding complications from a general anesthetic and also helping to avoid a costly hospital stay. Since the patient is awake during the procedure, they can sit or stand during the procedure to allow the surgeon to monitor the status of the patient and the overall progress of the contouring more effectively.

   Safety of the procedure:
Any surgery has both benefits and risks associated with it. When performed by a qualified dermatalogic surgeon the tumescent liposuction procedure with local anesthetic is extremely safe and has very few side effects. Minor complications may include lumpiness, dimpling, skin irregularity, loose skin, numbness, infections and some scarring although rare, may occur, and can usually be corrected fairly easily.

   Post-operative procedures:
The local anesthesia remains effective in minimizing pain from the surgery for approximately 24 hours. Patients are usually generally alert and able to function. They do not usually suffer the same nausea, grogginess and the washed out feeling associated with the effects of general anesthesia. Patients are usually encouraged to walk immediately after the procedure to enhance the healing process. Patients can usually resume regular desk-work within a few days, but physical exercise can be resumed three to seven days after the procedure.
